This position will be based in Coconino County, AZ out of our clinic in Flagstaff, AZ.
Bilingual Spanish/English Speaking is required for this position.
The Well Woman HealthCheck Program helps low-income, uninsured, and underinsured women gain access to breast and cervical cancer screening and diagnostic services.
We are looking for a Patient Navigator that has an interest in guiding patients through the healthcare system to ensure timely screenings, diagnosis, and treatment.
The Patient Navigator must be computer savvy and enjoy working in a office environment completing and processing large amounts of paperwork and computer data entry with accuracy and within specific timeframes.
General Description:
The Patient Navigator facilitates breast and cervical cancer screening for uninsured, program eligible clients.
They are responsible for determining client eligibility, identifying allowable services and securing medical appointments for new and returning Well Women HealthCheck Program (WWHP) clients.
The Patient Navigator component includes monitoring and documenting screening results and case managing abnormal results appropriately.
Additionally, the Patient Navigator identifies and mitigates screening barriers clients may encounter and assists them in completing their screenings, diagnostics and initialization of treatment with reminders, education while ensuring Quality Core Indicators are met and/or exceeded.
Occasional outreach will need to be done in the community; however it should be pre-approved by the Program/Project Coordinator.
This position reports to the Mohave County Program/Project Coordinator.
Essential Duties:
Contact clients for annual follow-up by phone and by mailIntake and verify program eligibility of new and returning clientsRoute ineligible clients to other services such as the Market Place, Medicare or sliding fee servicesMaintain an accurate phone/client encounter log (internal and external referrals tab)Identify services for which each client is due and schedule appropriate medical appointmentsAssist with cancer screening and diagnostic education of clients and clinic patients, as neededVerify timely completion and return of enrollment paperwork and assist clients with completing paperwork as neededMake reminder calls to decrease no shows and reschedule no-shows as neededEnsure that medical staff have any paperwork and information needed for appointments scheduledTrack clients, appointments and screening & diagnostic outcomes in appropriate spreadsheet and/or database and follow-up with clients as neededSecure schedule template and assist with planning and implementation of screening events Help interpret for our clients as neededCommunicate well with team and medical providers to ensure high quality care for our clients and proper utilization of other grant fundingAsk questions when necessary and provide feedback about processes and areas for program improvement to the Program/Project Coordinator, teammates, contracted Providers, and community partnersWork with the Program/Project Coordinator to identify and take advantage of appropriate continuing education opportunities and opportunities for involvement on clinic committeesWillingness to perform other duties as assignedSkills/Position RequirementsBilingual, Spanish/English speaking requiredExcellent customer service skillsAttention to detail, and organization strategies (to effectively prioritize and meet deadlines)Must keep private health information confidential and comply with HIPPA guidelinesFlexibility to work an occasional weekend day or travel out of town for screening daysCompetency with Microsoft Office, specifically Word, Excel and OutlookAbility to schedule and use Electronic Medical Records in Centricity Ability to accurately enter and pull data from our FileMaker Pro databaseExcellent team skills including communication, support and strong work ethicProfessional verbal and written communication skills a mustAssociates Degree or equivalent related experiencePreferences:
Experience with databases, data entry and/or practice management systemsWork experience with confidential information (medical, HR, legal, financial, etc.
)Customer Service training or work experiencePay Amount:
$15.
15/HR +DOE.
Estimated Salary: $20 to $28 per hour based on qualifications.

The check will bounce and the victim is left responsible.
