The program is a partnership between the Arizona Coalition for Military Families, Arizona Department of Veterans' Services and U.

The goal is to identify needs, issues and barriers earlier to prevent individuals and families from going into crisis.
The Be Connected Case Manager is a community-based position on the frontline of working with service members, veterans and their families.
Be Connected Case Managers conduct outreach to engage individuals and families and connect them to information, resources and support.
This includes helping to assess needs, match to available resources, coordinate care and reduce barriers.
Additionally, Be Connected Case Managers may conduct trainings, participate in community events and conduct outreach on the program.
Skills/ Requirement:
Applicants must have a valid Driver's License and must have a clear 39 month driving record.
